It is a pretty bad picture and the fact that the figure should come with two different heads is throwing me off, but I believe it is from Series 12.

Actually, it's Spawn VI from Series 20: Spawn Classic Series 2. Click on photo #3 to see, I believe the mask & gun was the variant figure to the unmasked with malbolgia's head.
