Here is one report regarding why the plane might have gotten so close without notifying air traffic controllers:

Ummm... Actually on the trips I've been on lately when you are landing in Laguardia Airport the planes go right up the Hudson and you are right over Manhattan and see the entire city.....

Poor weather in the area for VFR flight (Visual Flight Rules)
Likely inadvertantly got into instrument weather conditions and lost control.
A top of the list pilot killer. VFR into instrument meteorological conditions.
