Jean was turned into a human bomb by a Skrull posing as Hank during Secret Invasion. She ingested what she thought was a formula to enhance her powers. When the Skrulls detonated her when ahe was giant-sized Thor spun Mjolnir to grander her to another dimension where her explosion would harm no one else.

Kitty Pryde is still alive. She never died but was shot into space in the body of a giant hollow bullet pointed at Earth. Magneto brought her back recently. Being intangible somehow saves her from death in space.

Which makes no sense, as in 'God Loves, Man Kills', she phases herself and Illyana through the ground and then has to come up quickly to breathe.

She has got to breathe, and she can't do that while she's phasing, so unless she can hold her breath now for days...

First he was shot saving Cap from the Punisher, then he fought(injuried by the bullet) with the Sinester Six-well five because the looney Goblin murdered the good Dock Ock when he refused to play along-

Eventually the lil Spidey beat them all and in a final showdown with the Goblin both died.

Was it ever really explained how in the X-Men #100 reboot how Psylocke and Jean Grey switched powers kind of sorta? I don't think it was ever fleshed out on why one is a telekenetic only and the other was a telepath only.
