She's changed publishers a few times but is now back with Pulido and his Coffin Comics.
So out of that she has this kind of cult status among comic fans. Shes gone through changes over the years but her fanbase has always been there.
I could get more detailed but I figure that answers your question.

And where's Evil Ernie now, or is he just a fond memory of a 90's indie comic fan?

The latest Previews actually just solicited a new Evil Ernie series from Dynamite that's supposed to start in December.
I also wanted to mention that Lady Death is often credited with being one of the primary characters who ushered in the "bad girl art style" that began in the 90's.

SSC NYCC 2021 6th Oct 2021
PCS - Lady Death 1/3 painted.
I have to admit, the mean portrait expression fails to match the might of the previous two 1/4 pieces from Sideshow Collectibles
Easy pass for me. I am sure 1/3 fans will lap this up.

Back in the early 2000's Chaos! Comics went bankrupt. Pulido sold most of his character licenses off to other companies. Lady Death went to Cross Gen, until they themselves folded and Pulido was able to get the license back. However Evil Ernie, Purgatory and some others went to Devil's Due, and when they went under the rights were purchased by Dynamite Entertainment.

Need to see more, but is an easy pass from what I can see so far. Wish the Clayburn Moore statue was taller, because it's the definitive LD for me.

The portrait gives me pause, too. From the neck down everything reads realistic, but from the neck up the face reads cartoony.
