This really concerns me as I'm getting my Psylocke head replacement Thursday. This doesn't take away from the frustration for you that you're dealing with, but it certainly makes one worry. SS has been a mess this year.

Holy sh!t, Chronos, that’s a hot mess! Good luck on the next round of replacements.
It’s incidents like this that SS should consider a $5 rewards credit. It’s not like it doesn’t take time to go thru the initial replacement request, then having to do it a second time because of their staff slack job is just ridiculous and unnecessary.

I can’t say it surprises me.
I ordered the P1 Batman Hush from SS and they OBVIOUSLY sent me a return in that it didn’t come in the original outer boxes, the foam packing wasn’t fully taped together, and 1 of the hand switch outs and the cape were broken inside. It really was appalling that somehow that item found it’s way back to the “ok to ship” stock.
