Well i thought he could access only one power at a time, like turning powers on and off when needed. So if hes fighting and had to keep up a specific power, he couldnt heal himself in time to clear up the scar.
unlike claire i would venture the guess, if he doesnt access the healing power before the scar fully heals it becomes part of him. Then the healing power wont heal it, cause its already healed.
Or this future peter lost his power at some point, got cut up, healed and then got his powers back. In case wouldnt heal the scar only future injuries.
Claire on the other hand is always healing her wound, so she could never get a scar.
Just another theory.
